Job summary

Twin Cities Physicians in Minnesota is seeking a Next-Level Wound Care Nurse Practitioner (AGNP/CNP) to join our care teams inside assisted living, memory care, and long-term care settings. Expect diverse patients, advanced wound management, and collaborative practice with physicians and interdisciplinary teams.

This full-time role offers a generous compensation package ($120,000–$160,000+ total, plus mileage reimbursement) with full benefits, PTO, CME allowance, and malpractice coverage,

Qualifications

• Active Minnesota CNP license (AGNP preferred).
• Experience in wound care, geriatrics, primary care, post-acute care, or skilled nursing.
• Strong clinical judgment and ability to manage medically complex patients.
• Valid driver's license and ability to travel throughout the Twin Cities Metro Area.

Responsibilities

• Provide advanced wound care assessments, treatment planning, and ongoing management.
• Conduct routine rounding visits throughout assisted living, memory care, and long-term care communities.
• Manage medications, diagnostics, treatment plans, and follow-up care.
• Collaborate with physicians, nurses, therapists, caregivers, and facility teams.
• Support chronic disease management, preventative care initiatives, and continuity of care.
• Help improve healing outcomes while reducing avoidable complications and hospitalizations.
