Job summary

Twin Cities Physicians is seeking a Next-Level Wound Care Nurse Practitioner (AGNP/CNP) to deliver advanced wound care inside assisted living, memory care, and long‑term care settings across the Twin Cities.

This full‑time role offers a $120,000 to $160,000+ total compensation package, mileage reimbursement, comprehensive benefits, CME allowance, malpractice coverage, and a collaborative, interdisciplinary team focused on improving healing outcomes.

Qualifications

• Active Minnesota Certified Nurse Practitioner (CNP) license required, AGNP preferred.
• Experience in wound care, geriatrics, primary care, post-acute care or skilled nursing settings preferred.
• Strong clinical judgment and confidence in managing medically complex patients.
• Valid driver''s license and ability to travel throughout the Twin Cities Metro Area.

Responsibilities

• Provide advanced wound care assessments, treatment planning, and ongoing management.
• Conduct routine rounding visits throughout assisted living, memory care, and long-term care communities.
• Manage medications, diagnostics, treatment plans, and follow-up care.
• Collaborate with physicians, nurses, therapists, caregivers, and facility teams.
• Support chronic disease management, preventative care initiatives, and continuity of care.
• Help improve healing outcomes while reducing avoidable complications and hospitalizations.
