Job summary

CareMed Health Solutions seeks a Wound Care Nurse Practitioner to deliver wound management for residents in skilled nursing facilities. You will diagnose, treat, and manage wounds, collaborating with an interdisciplinary team and primary physicians.

The role requires an Ohio NP license, board certification, and experience with wound care and EMR systems. DEA license optional; prior long-term care experience preferred; strong communication essential.

Qualifications

• Current, unrestricted NP license in Ohio.
• Board certification as a Nurse Practitioner.
• Prescriptive authority with OH Law compliance.
• Experience in wound care, especially in geriatric settings.

Responsibilities

• Provide wound care services for long-term care patients in skilled nursing facilities.
• Prescribe medications, including controlled substances per Ohio law.
• Order and review treatments and diagnostics; follow up as needed.
• Communicate changes in condition with nursing home staff and care team.
• Collaborate with the primary care physician.
