Job summary

CareMed Health Solutions is seeking a Wound Care Nurse Practitioner to deliver wound services to patients in long-term care facilities. You will diagnose and treat wounds, prescribe medications, and coordinate with the interdisciplinary team to implement care plans.

This role requires a Kentucky NP license, board certification, and experience in geriatric wound care, with proficiency in using EMR systems and wound care products. Collaborative care with PCPs and facility staff is essential.

Qualifications

Current, unrestricted NP license in Kentucky.
Board Certification.
DEA licensure (Optional).
Clinical background in adult, family, or geriatric specialties.
Clinical background of wound care (chronic, debilitating, stalled, or infected wounds).
Comfort diagnosing and treating diabetic, arterial, venous, and mixed etiology wounds or Kennedy Ulcers.
Staging pressure injuries per the NPIAP.
Familiar with wound care products (collagens, alginates, foams, silvers).
Experience with wound VACs and other DME (pressure-relief devices).
Wound certification preferred or willingness to obtain.

Responsibilities

Provide wound care services for long-term care patients in skilled nursing facilities.
Prescribe medications, including controlled substances per KY law.
Order, review, and follow up with treatments and diagnostic tests.
Communicate with nursing home staff regarding changes in condition and care plans.
Collaborate with the primary care physician to coordinate patient care.
