Job summary

Hackensack Meridian Health (HMH) seeks an Advanced Practice Nurse (APN) to provide patient-centered, coordinated care across the care continuum in New Jersey. This role emphasizes leadership, clinical practice, consultation, and education, with a preference for wound care certification.

The APN will collaborate with physicians and staff, perform comprehensive assessments, manage pharmacologic therapies, and contribute to quality improvement initiatives while adhering to NJ regulations and HMH

Qualifications

• Graduate of NLN/AACN accredited nursing program.
• Master's or Doctorate degree from an accredited Nurse Practitioner program.
• Maintains clinical competencies through continuing education and practice.
• Experience in a relevant patient care setting per NJ regulations.
• NPI and CDS/DEA prescriptive authority as per NJ law.
• Collaborative agreement with physicians on staff.
• Excellent written and verbal communication skills.
• Proficient computer skills incl. MS Office/Google Suite.

Responsibilities

• Performs accurate history and physical assessments appropriate for patient age and clinical problem.
• Orders, performs, and interprets labs and diagnostics with physicians.
• Prescribes medications safely per best practices and NJ regulations.
• Promotes APN practice recognition through conferences, publications and presentations.
• Ensures patient- and family-centered care and adherence to policies and standards.
• Applies evidence-based medicine to improve patient outcomes.
• Participates in organizational meetings and interdisciplinary rounds.
