Job summary

Hackensack Meridian Health Inc. is seeking an Advanced Practice Nurse (APN) to provide patient-centered, coordinated care within a collaborative hospital team.

The APN will lead clinical practice, offer consultation and guidance, and support ethical decision-making across health and safety needs. The role emphasizes prescriptive authority under NJ law, ongoing professional development, and strong communication skills to coordinate care with physicians and staff while maintaining high standards

Qualifications

• Graduate of an NLN/AACN accredited nursing program.
• Master's or Doctorate degree from an accredited Nurse Practitioner program.
• Maintains clinical competencies through continuing education and clinical practice.
• Experience in a relevant patient care setting (or as regulations dictate).
• National Provider Identifier (NPI).
• Prescriptive authority through CDS and DEA registration according to New Jersey state law.
• Collaborative agreement with physicians on staff.
• Excellent written and verbal communication skills.
• Proficient computer skills that include but are not limited to Microsoft Office and/or Google Suite platforms.]

Responsibilities

• The APN utilizes a patient-centered coordinated care model, collaborating with the practice/hospital team to assess, plan, and implement care for individuals with health and safety needs.
• The APN will demonstrate leadership in direct clinical practice, consultation, coaching, and ethical decision-making to improve patient outcomes.
