Job summary

Hackensack Meridian Health (HMH) is seeking an Advanced Practice Nurse (APN) to deliver patient-centered, coordinated care. The APN will lead and collaborate with the care team, perform advanced assessments, and ensure adherence to NJ regulations and hospital policies.

A strong focus on education, ethics, and care transitions is expected. The ideal candidate holds an NLN/AACN accredited program and a Master’s or Doctorate in an NP program, with active NPI, CDS/DEA, and physician collaboration.

Qualifications

• Graduate of an NLN/AACN accredited nursing program.
• Master's or Doctorate degree from an accredited Nurse Practitioner program.
• National Provider Identifier (NPI).
• Prescriptive authority through CDS and DEA registration according to New Jersey state law.
• Collaborative agreement with physicians on staff.
• Excellent written and verbal communication skills.
• Proficient computer skills that include Microsoft Office and/or Google Suite.

Responsibilities

• Performs and documents accurate, complete histories and physical assessments.
• Orders, performs, and interprets laboratory tests and diagnostic studies.
• Manages general medical/surgical conditions with pharmacologic knowledge.
• Prescribes medication safely and provides patient education.
