Job summary

The Healogics Wound Care Center is seeking a licensed practical nurse to provide wound-related patient care under the direction of a Registered Nurse or Provider in an outpatient setting.

You will perform basic nursing duties, assist in vein clinic and intake/discharge, and follow hospital policies while delivering compassionate care to patients with chronic wounds. Hyperbaric therapy duties may be required after training.

Qualifications

• High School Diploma or GED.
• Graduate from an accredited practical nursing program/state approved educational program.
• Current LPN/LVN license in the state of practice.
• Two or more years’ experience as LPN/LVN, including SNF or LTACH experience.
• Wound care experience preferred.

Responsibilities

• Provide wound-related patient care under the direction of the RN/Provider and hospital policies.
• Assist in vein clinic as assigned after training.
• May assist with intake and discharge.
• May provide case management assistance under the direction of RN, per state regulations.
• Gathers patient information and necessary equipment.
• Reports quality of care issues to the Clinical Nurse Manager.
• Communicates with team regarding therapeutic interventions.
• Performs hyperbaric oxygen therapy related duties after training.
• May perform hyperbaric safety director duties after training.
• May function as a Documentation Assistant (scribe).
• Performs other duties as delegated.
