Job summary

Healogics is seeking a Licensed Practical Nurse (LPN) to support the Wound Care Center under RN or Provider guidance. Responsibilities include wound care, vein clinic assistance, patient intake, and discharge processes within a hospital setting.

Qualifications include a current LPN/LVN license, 2+ years of LPN/LVN experience (SNF/LTACH preferred), and BLS certification. Wound care experience is preferred; adherence to policies is required.

Qualifications

• High School Diploma or GED required.
• Graduate from an accredited practical nursing program/state approved educational program.
• Current LPN/LVN license in the state of practice.
• Two or more years’ experience as LPN/LVN (SNF/LTACH preferred).
• Wound care experience preferred.

Responsibilities

• Under RN/Provider direction, provide wound-related patient care.
• Assist in vein clinic after Vein Clinic training.
• May assist with intake and discharge.
• May support case management under RN direction.
• Gathers patient information and equipment.
• Reports quality issues to Clinical Nurse Manager.
• Communicate with team members regarding interventions.
