Job summary

Healogics is the largest provider of advanced wound care in the United States. The LPN will deliver wound care under RN or provider supervision in our center settings, following hospital policies and CMS guidelines.

Essential duties include assisting with vein clinics, intake and discharge, and teamwork to maintain high quality patient care. Requires state LPN/LVN license and at least 2 years’ experience, wound care preferred.

Qualifications

• High School Diploma or GED.
• Graduate from an accredited practical nursing program/state approved educational program.
• Current LPN or LVN license in the state of practice and in good standing.
• Two or more years of LPN/LVN experience, including 1+ year in SNF or LTACH setting.
• Wound care experience preferred.

Responsibilities

• Provide wound-related patient care under RN/Provider guidance.
• Assist in vein clinic as trained.
• Assist with intake and discharge.
• Support case management under RN direction where allowed by state regulations.
• Gather patient information and required equipment.
• Report quality of care issues to the Clinical Nurse Manager.
• Communicate with team members regarding therapies to ensure quality of care.
• Perform hyperbaric therapy-related duties after training.
• Possibly act as Documentation Assistant per policy.
• Perform other duties as assigned.
