Job summary

Healogics is seeking a Wound Care Center LPN to deliver wound-related patient care under the supervision of an RN or provider. You will help with vein clinic tasks, intake/discharge, and case management within a hospital-based wound care setting.

The role requires an active LPN/LVN license, a high school diploma or GED, and 2+ years of LPN/LVN experience, preferably in SNF or LTACH environments. Hyperbaric therapy duties may be assigned after training.

Qualifications

• High School Diploma or GED required.
• Graduate from an accredited practical nursing program/state approved educational program.
• Current LPN/LVN license in the state of practice.
• Two+ years’ experience as LPN/LVN, including SNF/LTACH setting preferred.

Responsibilities

• Provide wound-related patient care under RN/Provider direction.
• Assist in vein clinic after Vein Clinic training.
• May assist with intake and discharge.
• May support case management under RN guidance.
• Gather patient information and necessary equipment.
• Report quality of care issues to Clinical Nurse Manager.
• Communicate with team about therapeutic interventions.
• Perform hyperbaric therapy-related duties after training.
