Job summary

Healogics is the largest provider of advanced wound care services in the United States, treating more than 300,000 chronic wound patients annually across over 600 sites. The Wound Care Center LPN delivers wound-related and basic nursing care under RN direction, following hospital policies and CMS guidelines.

Under supervision, the LPN supports vein clinic tasks, intake/discharge, and case management as needed, while maintaining strong communication with the team to ensure high-quality patient

Qualifications

• High School Diploma or GED required.
• Graduate from an accredited practical nursing program/state approved educational program.
• Current LPN or LVN license in the state of practice.
• Two or more years’ experience as LPN/LVN, including at least one year in SNF or LTACH setting.
• Wound care experience preferred.

Responsibilities

• Provide wound-related patient care under RN/provider guidance per BLS and protocols.
• Assist in vein clinic after Vein Clinic training.
• May assist with intake and discharge.
• May assist case management under RN direction.
• Gather patient information and equipment, report quality issues to manager.
• Communicate with team regarding interventions to ensure quality care.
• Perform hyperbaric therapy duties as trained.
• Possibly serve as a scribe as per policy.
• Perform other duties as delegated.
