Job summary

Healogics is the leading provider of advanced wound care in the United States. The Wound Care Center LPN works under an RN or provider to deliver wound-related nursing care in a fast-paced outpatient setting.

You will support vein clinic tasks, intake/discharge, case management, and documentation while adhering to policies and CMS guidelines. Ideal candidates have an LPN license and 2+ years of LPN/LVN experience, with wound care background preferred.

Qualifications

• High School Diploma or GED.
• Graduate from an accredited practical nursing program/state approved program.
• Current LPN or LVN license in the state of practice - to maintain throughout employment.
• 2+ years’ experience as LPN/LVN, including 1+ years in SNF or LTACH setting.
• Wound care experience preferred.

Responsibilities

• Provide wound-related patient care under RN/Provider direction in accordance with policies and guidelines.
• Assist with vein clinic tasks after specialized training.
• Participate in intake and discharge processes as assigned.
• Support case management under RN supervision in compliance with state regulations.
• Record and report quality of care issues to Clinical Nurse Manager.
• Perform hyperbaric therapy-related duties after training and safety oversight as required.
• Serve as a Documentation Assistant (scribe) per policy.
• Collaborate with team to ensure timely, high-quality patient care.
