Job summary

Kindred Hospital Brea seeks an experienced Wound Care Coordinator to lead the wound care program and provide direct patient care. You will support staff nurses and act as a consultative resource for affiliated agencies.

In this role you will establish standards of care, educate teams on wound management, and collaborate with physicians to optimize patient outcomes in a California hospital setting.

Qualifications

• Must read, write and speak fluent English.
• Effective communication with hospital staff and patients.
• Ability to lead, motivate, and develop others.
• Program management skills for wound care initiatives.
• Knowledge of wound care guidelines (NPIAP/AHCPR/WOCN/CDC).

Responsibilities

• Develops and leads the facility wound care program with standard protocols.
• Provides direct patient care and supports staff nurses.
• Consults with physicians and affiliated agencies on wound care plans.
• Educates staff on products and wound care procedures.
• Reviews patient records and documents wound assessments and plans.
• Conducts RCA and contributes to performance improvement initiatives.
• Participates in CMS data reporting and formulary management.
