Kindred Hospital Houston Medical Center and Acute Rehabilitation Unit (ARU) seeks a Wound Care Coordinator to lead the wound care program and provide direct patient care alongside staff nurses. You will consult with physicians, develop care plans, and ensure adherence to wound care guidelines.

Requirements include an RN license, BSN preferred, BLS, and 1–3 years in acute care with wound care knowledge. Certification within 12 months is expected.

Competenze

• Degree from an accredited nursing program.
• Current state RN license; BSN preferred; BLS required.
• WOCN CWS or WCC certification, or obtain within 12 months of employment.
• 1–3 years of licensed professional nursing experience with acute care background.
• Knowledge of wound care per NPIAP/AHCPR guidelines.

Mansioni

• Develops and implements the facility wound care program in line with national standards.
• Establishes standards of care, policies and procedures for wound care staff.
• Acts as primary consultant to wound care clinicians and facility staff.
• Provides ongoing staff education on wound care products.
• Evaluates wounds on admission and during care to determine treatment plans.
• Provides wound care consultation for pressure injuries and non-healing wounds.
• Assists in plan of care development with physicians for comprehensive wound management.
• Reviews medical records to ensure proper documentation of wound assessments and treatments.
• Investigates wound-related adverse events with RCA and action planning.
• Participates in CMS data reporting and quality improvement initiatives.
• Collaborates with Support Center to maintain cost-effective wound formulary.
• Assists with training of new Wound Care Coordinators.
