Job summary

Kindred Hospital Riverside is seeking a Wound Care Coordinator to lead the facility wound care program and provide direct patient care while supporting nursing staff. You will serve as a consultant to clinicians, develop care protocols, and participate in quality improvement initiatives.

The role requires nursing licensure with 1-3 years of acute care experience, WOCN/WCC certification preferred or to be obtained within 12 months. English fluency and strong communication are essential.

Qualifications

• Must read, write and speak fluent English.
• 1-3 years licensed professional nursing experience with acute care background.
• Certification in WOCN/ WCC/ CWN is preferred or to be obtained within 12 months.

Responsibilities

• Develops and implements the facility wound care program per national standards.
• Establishes standards of care, competencies, policies and procedures for wound care staff.
• Act as primary consultant to Wound Care Clinicians and provide training to staff.
• Provides ongoing education on wound care products used in hospital.
• Assesses wounds on admission and during care to guide treatment plans.
• Consults on pressure injuries and non-healing wounds after two weeks.
• Assists physicians and nurses in developing care plans for wound management and prevention.
• Recommends changes to wound care orders with evidence-based support.
• Reviews patient records to ensure wound assessments and treatments are documented.
• Investigates adverse wound events and participates in RCA with nursing leadership.
• Participates in CMS wound-related data reporting and quality improvement initiatives.
• Collaborates to maintain cost-effective wound care formulary and assists training of new staff.
