Job summary

Extended Care LLC in Illinois seeks a dedicated Wound Care Coordinator to oversee wound care management and skin integrity programs for residents. The RN or LPN will provide high-quality wound care and ensure preventative measures align with regulations and facility policies.

You will conduct rounds, document findings, coordinate with physicians and interdisciplinary teams, and participate in care planning to support healing and safety in a compassionate, resident-centered environment.

Qualifications

• Active nursing license in good standing
• Current CPR certification
• Minimum 2 years as RN or LPN
• Ability to lift/move up to 75 lbs
• Strong communication in English

Responsibilities

• Oversee wound and skin care management programs
• Conduct regular wound care rounds and document findings
• Coordinate with physicians and interdisciplinary teams
• Implement preventative wound care strategies within care plans
• Maintain infection control and safety standards
