Job summary

Fora Health Treatment & Recovery seeks a withdrawal management nurse to provide comprehensive care for individuals undergoing withdrawal from substances. You will assess patients, develop plans, administer medications, and monitor progress within a multidisciplinary team.

Responsibilities include documenting care, educating patients, and ensuring safety during withdrawal with supervision from licensed providers. This role includes night shifts and collaboration with nursing assistants.

Qualifications

• Must have graduated from an accredited nursing program.
• Active nursing license or able to obtain within 8 weeks in Oregon.
• BLS certification required.
• Ability to pass pre-employment drug tests and maintain standards.

Responsibilities

• Assess patients’ health, including withdrawal history and vitals.
• Develop and implement individualized treatment plans with a multidisciplinary team.
• Administer medications and monitor responses with medical provider oversight.
• Document progress, vitals, and treatments accurately in records.
• Educate patients about withdrawal, options, and relapse prevention.
• Lead nursing assistant staff and ensure policy compliance.
