Job summary

Bridgeway Community Health in Salem, Oregon is seeking a Full-time Licensed Practical Nurse for the NOC shift to join our Medically Managed Withdrawal treatment team.

The successful candidate will provide direct nursing care, administer medications, document thoroughly, conduct nursing assessments, coordinate with physicians, and support discharge planning in a safe, patient‑centered environment.

Qualifications

• Graduation from a state-accredited School of Nursing program.
• A current and unencumbered license to practice as a Licensed Practical Nurse in Oregon.
• One year of LPN experience preferred; new graduates considered.
• Equivalent combination of education, training and experience acceptable.

Responsibilities

• Administers medications and treatments under direction of a nurse practitioner or physician.
• Completes medical documentation and charts in client records.
• Performs nursing assessments and documents health status; reports findings as needed.
• Prepares and dispenses medications; monitors interactions and side effects.
• Maintains prescription records and follows regulatory requirements for documentation.
• Coordinates client appointments with physicians and other agencies.
