Job summary

Responsibilities

• Collaborate with wellness program manager, health coaches, chronic condition nurses, and other team members to support participant success.
• Perform general administrative duties.
• Assist with onboarding new patients and program participants.
• Support chronic condition management programs through participant outreach and engagement.
• Provide assistance with web and app initiatives of wellness and educational programs.
• Maintain accurate participant records and documentation.
• Respond to participant questions and provide excellent customer service.
