Endeavors Inc. is seeking an Intake Coordinator/Outreach Specialist in New York to support the CBP Workforce Wellness Program. You will coordinate community outreach, foster partnerships, and engage participants, serving as a key program representative to promote services and access to wellness resources.

You will plan outreach events, manage referrals, and maintain accurate records while collaborating with the Program Manager to monitor engagement outcomes and ensure timely reporting.

Qualifications

• Bachelor’s degree preferred or equivalent experience.
• Experience in outreach, case management or service delivery is a plus.
• Strong communication and relationship-building skills.

Responsibilities

• Conduct initial eligibility screenings for individuals and families seeking program services.
• Develop and maintain collaborative relationships with community organizations and referral partners.
• Provide information regarding services and connect individuals to internal or community resources.
• Maintain participant records and intake documentation.
• Coordinate referrals and connect individuals to wellness, case management, and related services.
• Monitor engagement and address barriers to participation as appropriate.
• Prepare reports on outreach activities, referral trends, and participant engagement.
• Represent the organization at community events and outreach activities.
