Job summary

Endeavors Inc. is seeking a Case Manager to promote the CBP Workforce Wellness Program, screen eligibility, and coordinate access to wellness services for CBP employees and their families.

You will be a primary contact ensuring clients receive resources that enhance wellness, resilience, recovery, and quality of life. As part of a multidisciplinary team, you will document accurately, adhere to policies, and coordinate with clinicians and external providers to deliver coordinated services in

Qualifications

• Bachelor’s degree in social work, sociology, psychology, or related field preferred or equivalent experience.
• 2–4 years of case management experience preferred.
• Experience with law enforcement, veterans and their families is a strong plus.

Responsibilities

• Coordinate referral, authorization, and invoicing procedures with external providers.
• Assess client needs and develop comprehensive wellness plans.
• Coordinate, monitor and document service delivery per the wellness plan.
• Advocate for client access to wellness services and build community partnerships.
• Provide wellness-focused case management to law enforcement personnel and eligible family members.
• Review and process invoices; verify provider credentialing status.
• Serve as liaison between clients and external providers to ensure quality service.
• Demonstrate exceptional customer service and support the mission to empower lives.
