Job summary

Endeavors is seeking a Case Manager to promote the CBP Workforce Wellness Program, screen eligibility, and coordinate access to wellness services for employees and their families. You will assess needs, develop wellness plans, coordinate referrals, and liaison with clinicians and providers to ensure coordinated care in office-, community-, or virtual-settings.

This role requires accurate documentation, adherence to policies, and willingness to travel.

Qualifications

• Bachelor’s degree in social work, sociology, psychology, or related field.
• 2–4 years of case management experience; 2+ years in a customer service-focused environment.
• Experience with law enforcement, veterans & their families is a strong plus.
• Driver’s License with clear record; must pass a government background investigation.
• Must be available to travel to various locations as needed.

Responsibilities

• Coordinate referral, authorization, and invoicing procedures with external providers.
• Assess client needs and develop comprehensive wellness plans.
• Coordinate, monitor and document service delivery with accurate case records.
• Advocate for client access to wellness services and resources.
• Provide wellness-focused case management to law enforcement personnel and families.
• Review, verify, and process external invoices and ensure compliance.
• Monitor provider credentialing status to ensure qualified providers.
• Serve as liaison between clients and external providers for coordinated care.
• Perform other duties as assigned.
