Qualifications

• Bachelor's degree in Social Work or related field required.
• 1–3 years in case management, client intake, care coordination, behavioral health, or related human services settings.
• Driver’s License with clear record and ability to pass a government background investigation.
• Willingness to travel to various locations as needed.

Responsibilities

• Conduct intake interviews and eligibility screenings for prospective program participants.
• Gather, review, and verify client information and supporting documentation to determine program eligibility.
• Provide program information, guidance, and referrals when eligible or when enrollment is limited.
• Identify client strengths, needs, barriers, and service priorities through assessments.
• Develop individualized wellness plans with clients, setting goals and support strategies.
• Coordinate and monitor service delivery to address client needs.
• Provide ongoing case management and follow-up to support engagement and outcomes.
• Facilitate referrals and coordinate care with clinicians and community resources.

Education

Bachelor's degree in Social Work, Counseling, Psychology, Human Services, Public Health, or related field
