Job summary

Endeavors is seeking a Case Manager/Intake Coordinator to assess eligibility and enroll CBP employees and eligible family members into the CBP Workforce Wellness Program in Detroit, MI. This full-time role requires up to 50% travel and a Bachelor's degree in a related field.

Qualifications

• Bachelor's degree required or equivalent combination of education and experience.
• 1–3 years of case management or related human services experience.
• Driver's license with clean record; willing to undergo background check.
• Attendance must be regular and acceptable as determined by employer.
• Must be able to travel to various locations as needed.

Responsibilities

• Conduct intake interviews and eligibility screenings for prospective program participants.
• Gather, review, and verify client information and documents to determine eligibility.
• Provide information, guidance, and referrals when enrollment is not possible or capacity is reached.
• Identify client strengths, needs, barriers, and service priorities through assessments.
• Develop individualized wellness plans with goals and recommended services.
• Coordinate and monitor service delivery to address client needs effectively.
• Provide ongoing case management and follow-up to support engagement and outcomes.
• Facilitate referrals and coordinate care with clinicians and external resources.
• Conduct behavioral health screenings when appropriate to determine level of care.
• Coordinate initial behavioral health appointments and ensure timely access.
• Collaborate with multidisciplinary team to address client concerns and safety risks.
• Represent the program at outreach and community events; deliver presentations.
