Job summary

Endeavors seeks a Case Manager/Intake Coordinator to assess eligibility and enroll CBP employees and eligible family members into the CBP Workforce Wellness Program. This role will serve as a key contact, conduct comprehensive eligibility screenings, and ensure timely access to program resources.

Qualifications

• Bachelor's degree in Social Work or related field required.
• 1–3 years of experience in case management, client intake, care coordination, behavioral health, or related settings.
• Travel up to 50% and flexibility in schedule.

Responsibilities

• Conduct intake interviews and eligibility screenings for prospective participants.
• Develop individualized wellness plans with clients and establish goals.
• Coordinate and monitor service delivery to address client needs.
• Advocate for clients and coordinate with clinicians and community resources.
• Maintain accurate case notes and documentation.
