Job summary

Endeavors Inc. in Detroit seeks a Case Manager/Intake Coordinator to assess CBP employees and eligible family members for the Workforce Wellness Program, conducting eligibility screenings and enrolling participants.

You will work with a multidisciplinary team to coordinate care, develop individualized wellness plans, advocate for access to services, and maintain confidentiality while managing comprehensive case records.

Qualifications

• Bachelor’s degree in Social Work, Counseling, Psychology, Human Services, Public Health or related field is required.
• 1–3 years of experience in case management, client intake, or related human services settings preferred.
• Experience with law enforcement, veterans & their families is a strong plus.

Responsibilities

• Conduct intake interviews and eligibility screenings for prospective program participants.
• Gather, review, and verify client information to determine program eligibility.
• Provide program information, guidance, and referrals when eligible status is unclear or enrollment is limited.
• Develop individualized wellness plans with clients, setting goals and service priorities.
• Coordinate and monitor service delivery to ensure client needs are addressed effectively.
• Provide ongoing case management and follow-up to support engagement and outcomes.
• Facilitate referrals and coordinate care with clinicians and external resources.
• Maintain accurate case records and ensure privacy and compliance with standards.
