Qualifications

Responsibilities

• Build trust-based relationships with patients from first interaction.
• Guide patients through wellness offerings and enrollment options.
• Support patient engagement, retention, and treatment plan participation.
• Coordinate follow-ups, scheduling and patient communications.
• Create a seamless, elevated patient experience at every touchpoint.
• Track KPIs for engagement, follow-up and enrollment outcomes.
• Support new wellness initiatives and service launches.
• Partner with providers and clinic staff in a collaborative environment.
