Responsibilities

• Build trust-based relationships with patients from first contact
• Guide patients through wellness offerings and align with goals
• Support patient engagement, retention, and treatment plan participation
• Coordinate follow-ups, scheduling, and patient communications
• Create a seamless, elevated patient experience at all touchpoints
• Track KPIs related to engagement and enrollment in new services
• Support wellness initiatives and service launches
• Collaborate with providers and clinic staff
