NIVA Health is seeking a Wellness Case Manager to guide patients through their wellness journey—from initial conversation to ongoing engagement with treatment plans. You will work closely with clinicians to keep patients informed, engaged, and connected to services that meet their health goals.

Qualifications

• Track record of guiding people toward a purchase or enrollment in wellness or related fields.
• Personable, energetic, and confident in one-on-one conversations.
• Excellent communication and quick rapport-building.
• Organized, proactive with follow-up and able to manage multiple patients.
• Comfortable in a performance-driven yet supportive environment.

Responsibilities

• Build trust-based relationships with patients from first interaction.
• Guide patients through wellness offerings and enrollment options.
• Support patient engagement, retention, and treatment plan participation.
• Coordinate follow-ups, scheduling, and patient communications.
• Create a seamless, elevated patient experience at every touchpoint.
• Track KPIs for engagement and enrollment in new services.
• Assist with new wellness initiatives and launches within the clinic.
• Collaborate with providers and clinic staff in a team setting.
