Job summary

NIVA Health is seeking a Wellness Case Manager to guide patients through their wellness journey from the first conversation through ongoing treatment plan engagement. This on-site, full-time role sits at the heart of our clinic—driving patient engagement, enrollment outcomes, and meaningful health outcomes.

You will partner with providers and clinic leadership to keep patients informed, engaged, and connected to the services that support their health goals, delivering an elevated patient

Qualifications

• Track record of guiding people toward a purchase or enrollment in wellness or health programs.
• Excellent interpersonal communication and rapport-building.
• Ability to manage multiple patient relationships with attention to detail.

Responsibilities

• Build trust-based relationships with patients from their first interaction.
• Guide patients through wellness offerings aligning with goals.
• Support engagement, retention, and treatment plan participation.
• Coordinate follow-ups, scheduling, and patient communications.
• Track KPIs for engagement and enrollment in new services.
• Collaborate with providers and clinic staff.
