- Direct, administer, and coordinate clinical resident care services
- Lead, manage, and maintain overall accountability for the team and performance
- Oversee Wellness revenue, evaluations, care conferences, and service plans
- Ensure the right care, at the right price, with the right staff
- Serve as Alzheimer and Dementia Program Coordinator
- Lead 1440 care standards
- Translate organizational, community, and wellness goals to team, staff, and residents
- Lead relationships with community leaders and external partners, including skilled care
- Market wellness throughout the community with the Executive Director and Sales Team
- Perform resident admission and ongoing evaluations every six months, after hospital or skilled-environment returns, or following changes in condition
- Set measurable resident goals and outcomes addressing physical, mental, and psychosocial needs
- Manage state regulations and auditing for community compliance
- Obtain written orders for changes to care plans
- Verify MARS/TARS and communicate with pharmacy regarding medication and treatment administration accuracy
- Review incident reports and determine whether assessments or care conferences are needed
- Address identified personnel issues
- Communicate emergency policies and provide updated information to staff
- Work toward continual organizational improvement
- Perform other duties as assigned
Requirements
- Licensed Practical Nurse (LPN) or Registered Nurse (RN) registered within the State preferred
- CDP required prior to or upon hire within 90 days
- Assisted Living/Memory Care experience preferred
- Leadership experience preferred
- Proven ability to manage revenue and financials successfully
- Proven high performer
- Ability to work successfully through complex issues and problem solve
- Available to work a flexible schedule including on call, weekends, and holidays when necessary
- Ability to stand for long periods, communicate effectively, sit, stand, walk, use hands for keyboard and other equipment, and review detailed documents and computer screens
- Occasionally lift and/or move up to 25 pounds
- Occasional travel to different locations may be required
