Job summary

Jobtailor is seeking an experienced licensed practical nurse to deliver direct nursing care to residents in Edgefield, SC. The role includes supervising nursing assistants, administering medications, and documenting care with accuracy, while coordinating with physicians, families, and the care team.

You will develop and update care plans, monitor patient status, and lead the nursing team in compliance with long-term care guidelines.

Qualifications

  • Requires Nursing Degree or LPN/LVN program completion.
  • Current, active LPN/LVN license in the state.

Responsibilities

  • Provide direct nursing care to residents.
  • Supervise day-to-day nursing activities by CNAs and nursing staff.
  • Administer medications and treatments.
  • Document resident care and required records accurately.
  • Coordinate nursing services with physicians, therapists, CNAs, residents, families, and departments.
  • Admit, transfer, and discharge residents.
  • Develop, implement, review, and update care plans.
  • Monitor residents and report changes, incidents, or discrepancies.
  • Perform nursing procedures (catheterization, tube feedings, suction, dressings, ROM).
  • Conduct rounds and ensure care plans are followed.
  • Lead and train nursing personnel.
  • Participate in QA, infection control, safety, and staff development.
  • Maintain medication, narcotic, equipment, and census records.
  • Support residents and families during admissions, orientation, discharge.

Job description


  • Provide direct nursing care to residents

  • Supervise day-to-day nursing activities performed by nursing assistants

  • Administer medications and treatments

  • Accurately document resident care and complete required records, charts, reports, and forms

  • Coordinate nursing services with physicians, therapists, CNAs, other nurses, residents, families, and support departments

  • Admit, transfer, and discharge residents

  • Develop, implement, review, and update care plans and discharge plans

  • Monitor residents' physical and emotional status and report changes, accidents, incidents, and discrepancies

  • Perform nursing procedures including catheterization, tube feedings, suction, dressing changes, colostomy and drainage-bag care, specimen collection, vital signs, and range-of-motion exercises

  • Conduct rounds, inspect care areas, and ensure treatments and care plans are followed

  • Lead, assign, train, evaluate, and supervise nursing personnel

  • Participate in quality assurance, facility surveys, infection control, safety, emergency preparedness, and staff development programs

  • Maintain medication, narcotic, equipment, supply, census, and compliance records

  • Provide resident and family support, including admissions, orientation, discharge coordination, and post-mortem procedures


Requirements


  • Nursing Degree from an accredited college or university, or graduation from an approved LPN/LVN program

  • Current, unencumbered, active license to practice as an LPN/LVN in the state

  • Current CPR certification

  • Must maintain required continuing education and licensing

  • Must remain in good standing with the State Board of Nursing

  • Knowledge and experience with PCC preferred

  • Ability to read, write, speak, and understand English

  • Ability to read technical procedures and policy and procedure manuals

  • Ability to communicate resident condition and respond to questions from managers and employees

  • Knowledge of nursing and medical practices, procedures, laws, regulations, and long-term-care guidelines

  • Leadership and supervisory ability

  • Ability to make independent decisions and work effectively with residents, families, personnel, visitors, and agencies

  • Ability to plan, organize, develop, implement, and interpret nursing programs, policies, procedures, and care objectives

  • Ability to move intermittently, work with ill, disabled, elderly, emotionally upset, and sometimes hostile people

  • Ability to push, pull, move, and/or lift at least 25 pounds to a height of 5 feet and carry it at least 50 feet

  • Must complete required medical and physical examination and meet facility health requirements

  • May need to assist with resident evacuation during emergencies
