Qualifications

• Proficiency with Microsoft Office and data reporting tools.

Responsibilities

• Provides a full range of community and social support services to an assigned caseload.
• Completes rehabilitation assessment and recovery plans with outcomes tracking.
• Engages clients to increase involvement in learning, work, and social activities.
• Recruits members through educational meetings/forums.
• Motivates participants in health and wellness programs.
• Teaches health management and provides individual health coaching.
