This position will provide community support and individual interventions based on assessment and evaluation; provide linkage and referral based on individual need and as required by funded agency programs in the Recovery and Empowerment Center. Responsibilities will primarily consist of the provision of direct care in a group setting; generally consist of guiding, assisting and supporting individuals who have complex psychiatric, substance abuse, medical, social and legal issues to improve daily living skills, improve level of functioning, enhance independence and promote recovery and community integration. Assist members/participants in finding new ways to inspire healthier habits through health and wellness. This position may include development and implementation of a full range of therapeutic, recreational, vocational and social activities. Hours may vary and may include evening, holidays, and weekends.

ESSENTIAL RESPONSIBILITIES
  • Provides a full range of community and social support services to an assigned caseload of individuals.
  • Completes timely rehabilitation assessment, reassessments and completes/updates recovery plans, as well as all tracking and reporting requirements for outcomes and evaluations.
  • Engages clients and works in partnership to increase or maintain involvement in learning, working and social activities.
  • Recruits members through educational meetings/forums.
  • Possesses a high level of ability to motivate participants in the health and wellness program.
  • Teaches members about managing their mental and physical health. Provides individual health coaching to participants in reaching health objectives.
  • Pursues best practice models for clients in continuing education, employment and socialization.
  • Develops relationships with clinical, educational, vocational and other social service agencies in order to effectively support clients in the community with increasing levels of independence.
  • Creates and adapts wellness objectives to overcome barriers to good mental and physical health based on needs of participants and with input from participants.
  • Provide continuous assessments, modifies interventions as appropriate and completes documentation, and all required paperwork based on funder/program/agency requirements.
  • Ensures provision of safety with all participants and equipment in the Fitness Center.
  • Provides transportation to clients as appropriate utilizing personal vehicle for agency business.
  • Attends and participates in all required meetings.
  • Adheres to all Chrysalis Center, Inc.’s policies, procedures and maintains professional standards at all times.
  • Performs other duties as assigned.
Qualifications
  • Bachelor’s degree required plus a minimum of one to two years of experience in mental health and substance abuse fields.
  • Bi-lingual Spanish, preferred.
  • A minimum of one year required with experience in personal training and health and wellness programming.
  • Certification in personal training or other national certification in group fitness instruction.
  • Experience in working with diverse populations in a group setting.
  • Proficiency managing Microsoft and other computer applications used for tracking data and reporting outcomes.
  • Easily adapts to change and competing priorities.
  • Ability to de-escalate crisis situation with professionalism and sound judgment.
  • Ability to effectively interact with clients and members of various teams within and outside the agency.
KEY COMPETENCIES

  • Maintains current fitness certification.
  • Understands and abides by all agency policies and procedures.
  • Completes and maintains documentation and required paperwork for all clients on time.

  • Able to solve practical problems and deal with a variety of variables in situations with limited standardization.
  • Proficient with use of computer i.e. Microsoft Office, Electronic Health Record, etc., filing skills and other administrative skills.
  • Skilled at individual and group based interventions.
  • Demonstrates excellent organizational, time management skills.
  • Effectively communicates and presents information clearly and in a concise manner in verbal and written format.
  • Demonstrates excellent interpersonal skills including professional demeanor and positive, service orientated attitude toward all internal and external customers.
  • Demonstrates ability to adhere to strict Confidentiality and HIPAA Guidelines.
  • Provides guidance, assistance and support to clients as well as works collaboratively with Chrysalis Team.
