Job summary

Highmark Inc. seeks a seasoned Wellness/Lifestyle Coach to supervise a panel of members, assess health management needs, and design personalized care plans with a multidisciplinary team.

You will engage members by phone, digital channels, or virtual visits, tracking outcomes and aligning interventions with evidence-based standards while ensuring regulatory compliance. Requires a Master’s degree in a health field and at least 3 years of relevant experience; certifications such as CDE or CHWC are

Qualifications

• Master's degree required or equivalent experience as determined by the company
• Minimum 3 years as Registered Dietitian, Exercise Physiologist, or Wellness/Lifestyle Coach
• Experience in wellness program delivery within health plans or disease management
• Certifications such as CDE or CHWC preferred

Responsibilities

• Oversee a specified panel of members and assess health management needs
• Create care plans addressing identified needs and remove barriers to care
• Coordinate referrals to internal and external resources
• Demonstrate clinical outcomes and cost savings from wellness interventions
• Document all activities in compliance with policies and standards
• Participate in workgroups to improve program efficiency and effectiveness
