Job summary

Reserve Care invites a Weekend Social Worker-Onsite Triage to join our Cleveland team, working Saturdays and Sundays from 8:00 am to 8:30 pm, totaling 24 hours/week but paid for 28 hours. This part-time role emphasizes psychosocial assessment, access facilitation, and timely referral resolution within our care navigation team.

Qualifications

• OH LSW licensure required.
• Hospice experience preferred.
• Ability to work effectively within an interdisciplinary team.

Responsibilities

• Assist with referral intake and processing through dispatch using multiple technology platforms.
• Support psychosocial assessment of complex and challenging referrals.
• Follow up on referrals to ensure access, quality, and timeliness.
• Participate in quality improvement and process improvement initiatives.
• Train peers on referral/intake/dispatch processes and end-of-life topics.
• Perform all other duties as assigned.
