Job summary

Reserve Care in Cleveland, OH is seeking a Weekend Social Worker-Onsite Triage to support psychosocial assessment, access facilitation, and one-call resolution for referrals. The schedule is Saturday and Sunday, 8:00am–8:30pm, totaling 24 hours per week but paid for 28 hours.

You will collaborate within an interdisciplinary team, participate in training peers on intake and end-of-life topics, and handle duties aligned with compassionate hospice care in a supportive environment.

Qualifications

• Licensed Social Worker (LSW) in Ohio.
• Hospice experience preferred.
• Ability to work effectively within an interdisciplinary team.

Responsibilities

• Assist with referral intake and processing through dispatch using multiple technology platforms.
• Support psychosocial assessment of complex and challenging referrals.
• Follow up on referrals to ensure access, quality, and timeliness.
• Participate in quality improvement and process improvement initiatives.
• Train peers on referral/intake/dispatch processes and end-of-life topics.
• Perform all other duties as assigned.

The schedule for this role is Saturday and Sunday, 8:00am-8:30pm. You will work a total of 24 hours a week and get paid for 28 hours a week.

The Care Navigation Social Worker supports referral intake and processing with a focus on psychosocial assessment, access facilitation, and one-call resolution.

WHAT YOU WILL DO:

• Assist with referral intake and processing through dispatch using multiple technology platforms.
• Support psychosocial assessment of complex and challenging referrals.
• Follow up on referrals as needed to ensure access, quality, and timeliness.
• Participate in quality improvement and process improvement initiatives.
• Participate in training peers on referral/intake/dispatch process as well as end-of-life related topics.
• Perform all other duties as assigned.

QUALIFICATIONS:

• Licensed Social Worker (LSW) in the state of Ohio
• Hospice experience preferred
• Ability to work effectively within an interdisciplinary team

PHYSICAL REQUIREMENTS AND WORKING CONDITIONS

This is sedentary work requiring the exertion of up to ten (10) pounds of force occasionally and some light work and lifting requiring the exertion of up to 20 pounds occasionally; work requires sitting, stooping, bending, kneeling, reaching, standing, fingering, grasping and repetitive motions; vocal communication; hearing, and visual acuity. Employee is not subject to adverse environmental conditions. This position has the potential to be exposed to environmental elements, internal and external, such as infectious disease, bloodborne pathogens, and chemical and/or chemical fumes, (pet dander and/or exposure to allergens) and adverse effects of inclement weather that may occur as well as potentially difficult patient and family situations.
