Job summary

Reserve Care in Cleveland, OH is seeking a part-time Care Navigation LPN to support referral intake, care coordination and initial clinical triage for hospice services. The role emphasizes one-call resolution and smooth progression of referrals through dispatch.

Qualified candidates are Ohio-licensed LPNs, with hospice experience preferred and strong communication skills. The position involves coordinating with care teams, EMR use, and direct admits as needed, offering meaningful patient support.

Qualifications

• Licensed Practical Nurse (LPN) in Ohio with current license.
• Hospice experience strongly preferred.
• Excellent verbal and written communication skills.
• Ability to communicate calmly and compassionately with patients and families.
• Strong teamwork and collaboration skills.
• Comfortable using EMR, phone systems, and referral/intake platforms.

Responsibilities

• Retrieve, monitor, and process referrals for hospice services from multiple sources.
• Learn and utilize the dispatch process to schedule appointments for prospective patients.
• Follow up on referrals that do not progress to dispatch timely.
• Perform direct admits to inpatient care centers as needed.
• Take clinical calls from patients and caregivers; resolve or escalate appropriately.
• Notify primary care teams of addressed clinical issues.
• Participate in quality and process improvement initiatives.
• Perform other duties as assigned.
