Job summary

Qualifications

• 1 year of experience in a clinical setting.
• 3 years of experience in an acute care clinical setting preferred.
• RN or Social Worker with required licensure and ACM/CCM/ANCC preferences.

Responsibilities

• Coordinate care for patients to support safe, timely transitions across the continuum.
• Identify and assess needs, plan and implement options with the healthcare team.
• Document plans of care and ensure accurate handoffs during care transitions.
• Support discharge planning and reduce unnecessary delays.
• Promote resources and accessibility for patient and caregiver needs.
