Job summary

Affinity Hospice seeks a registered nurse/case manager to plan, organize, and deliver hospice care, drawing on nursing experience with community health focus. Responsibilities include leading an interdisciplinary team, developing patient care plans, and ensuring compliant documentation to support high-quality care.

The role emphasizes home-based services, coordination with families, and ongoing professional development.

Qualifications

• Current State license as a Registered Nurse.
• CPR certification.
• Minimum of two years as an RN in clinical care/home health/hospice preferred.

Responsibilities

• Case manages and provides clinical direction to the interdisciplinary team, physician, and family.
• Implement and document the patient plan of care.
• Provide care with infection control measures to protect staff and patient (OSHA).
• Ensure continuity of quality patient care with proper documentation.
• Monitor cases for third‑party payer requirements.
• Supervise hospice aides and licensed practical nurses.
• Perform on‑call duties as assigned.
