Job summary

Affinity Hospice Holdings, LLC is seeking a Registered Nurse/Case Manager to plan, organize, and provide hospice care in home settings. The role emphasizes community health, education, and coordination with families to tailor plans of care.

Qualifications include a nursing degree, active RN license, CPR, and at least 2 years of RN experience in hospice or home health. The position is full-time with weekend on-call duties and requires a valid driver’s license.

Qualifications

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
• Current State license as a Registered Nurse (RN).
• Current CPR certification.
• Minimum of two years of RN experience in clinical care, home health, or hospice setting preferred.

Responsibilities

• Case manages and provides clinical direction to the interdisciplinary team, physician, and family.
• Implement and document the patient plan of care.
• Provide care with infection control measures to protect staff and patient.
• Ensure continuity of quality patient care with proper documentation.
• Monitor cases for third-party payer requirements.
• Supervise hospice aides and licensed practical nurses.
