Job summary

Reserve Care is seeking an on-call Registered Nurse (RN) or Licensed Practical Nurse (LPN) for weekend coverage in the East Campus area, Willoughby Hills, OH. The role focuses on delivering compassionate, skilled nursing care in patients' homes during after-hours periods.

You will assess needs, provide intermittent nursing services, manage pain, educate families, and coordinate with physicians. Ohio licensure, nursing degree, and strong communication are essential.

Qualifications

• Licensed RN in Ohio is required.
• Graduate of an accredited nursing program; BSN preferred.
• Experience in pain and symptom management, oncology, or mental health preferred.
• Strong written and verbal communication; able to work independently and with a team.

Responsibilities

• Assess patient and family needs (physical, psychosocial, spiritual).
• Provide intermittent skilled nursing per physician direction.
• Monitor pain and symptoms; relay information to physician and family.
• Serve as liaison between patient, family, and health professionals.
• Offer emotional support and health teaching in the home setting.
• Complete documentation promptly and accurately.
• Participate in team meetings and RC University trainings as needed.
• Occasional triage coverage as required by team leader.
