Reserve Care is seeking an On-Call Nurse to support patients and families in the community after hours. You will assess needs, provide skilled nursing under physician direction, and coordinate care with the team.

Qualifications

• High School Diploma or equivalent required.
• Graduate of an accredited school of nursing; BSN preferred.
• Licensed RN/LPN in Ohio.
• Experience preferred in pain and symptom management, oncology, or mental health.
• Effective communication, oral and written.
• Ability to work independently and with a team.
• Care for patients across all ages; good decision making under pressure.
• Valid driver's license with good driving record.

Responsibilities

• Assess patient and family needs (RN).
• Gather data on physical, psychosocial, and spiritual needs (LPN).
• Provide intermittent skilled nursing under physician direction.
• Monitor pain and symptoms; aid patient and family in management.
• Serve as liaison to physicians; relay patient information.
• Provide emotional support to patient and family.
• Refer to other services when appropriate.
• Provide health care teaching to patient and family.
• Visit patient residences at end of life when needed.
• Maintain and return required documentation.
• Communicate with patients, families, and health professionals with confidentiality.
• Participate in team meetings and mandatory events.
• RN to participate in triage coverage as needed.
• Perform other duties as assigned.
