Job summary

Reserve Care is seeking an On-Call Nurse to support patients and families after hours, including weekends. You will operate in patients’ homes, delivering skilled nursing care per physician directions and coordinating with the care team to manage pain and symptoms.

The role requires RN or LPN licensure in Ohio, strong communication, and the ability to work independently under pressure while maintaining compassion and confidentiality in home settings.

Qualifications

• High School Diploma or equivalent required.
• Graduate of an accredited school of nursing, with a Bachelor's degree preferred.
• Licensed RN/LPN in the state of Ohio.
• Experience preferred in pain and symptom management, oncology, or mental health.
• Ability to communicate effectively, orally and in writing.

Responsibilities

• Assess physical, psychosocial and spiritual needs of the patient and family (RN only).
• Provide intermittent skilled nursing services to the patient under the physician’s direction.
• Monitor and evaluate pain and symptoms; provide information to patient and family to manage those problems.
• Serve as a liaison to the patient’s physician(s) and provide information based on patient needs.
• Provide emotional support to the patient and family.
• Refer patients to other services when appropriate.
• Provide instruction to patient and family related to care at home.
• Make visits to patient homes, including end-of-life care when needed.
• Maintain and return all required documentation promptly.
