Job summary

Reserve Care (formerly Hospice of the Western Reserve) is seeking an On-Call Nurse to support patients and families in home settings. The role offers after-hours coverage, with responsibilities to assess needs, deliver intermittent skilled nursing, and coordinate with physicians while maintaining clear documentation.

Applicants should be an RN or LPN licensed in Ohio, with strong communication skills and the ability to work independently and with a team.

Qualifications

• Licensed RN or LPN in the state of Ohio.
• Experience in pain and symptom management preferred.
• Strong communication skills, written and verbal.
• Ability to work independently and with a team.
• Proficiency with Word and Excel; ability to use donor databases and grant portals.

Responsibilities

• Assess physical, psychosocial and spiritual needs of the patient and family (RN/LPN).
• Provide intermittent skilled nursing services under physician direction.
• Monitor pain and symptoms; assist patient and family in management.
• Communicate with physicians and team; maintain confidentiality and documentation.
• Provide education to patients and families about home care.
• Perform visits to patient residences; participate in triage coverage as needed.
• Maintain and promptly complete required documentation.
