Job summary

Reserve Care is seeking an On-Call Licensed Practical Nurse (LPN) to support weekend and after-hours home visits in Mentor, OH. The role emphasizes compassionate patient care in hospice, palliative, and grief support settings.

You will work Saturdays and Sundays, 8:00 am to 8:30 pm, providing skilled nursing services, symptom management, and caregiver education under physician direction. A valid OH LPN license is required.

Qualifications

• Current Ohio Licensed Practical Nurse (LPN) license.
• Experience in hospice, palliative care, or grief support is preferred.
• Ability to communicate effectively, both orally and in writing.
• Must be able to work independently and as part of a team.
• Able to handle stress and adapt to changing conditions.
• Must maintain valid driver’s license and auto insurance as required.

Responsibilities

• Assess nursing needs of patients and families in the home setting.
• Provide intermittent skilled nursing services under physician direction.
• Monitor pain and symptoms; educate patients and families on care and management.
• Coordinate with physicians and other health professionals as needed.
• Maintain accurate documentation and comply with policies and procedures.
