Job summary

South Texas Health System Edinburg is seeking a dedicated RN Case Manager to coordinate patient care, optimize resource utilization and support discharge planning across the hospital stay.

The role requires current Texas RN licensure, a basic nursing degree, and preferably BSN; bilingual Spanish is a plus. Candidates should have acute care experience and familiarity with care coordination, with weekend/evening/on-call coverage as needed.

Qualifications

• Licensed in Texas as RN with current license.
• Registered Nurse licensed in Texas or compact state or hold temporary TX license while awaiting permanent license.
• BSN preferred.
• Associate/Baccalaureate/Diploma from Nursing school or university required by Jan 2016.

Responsibilities

• RN Case Manager to coordinate resource utilization and clinical outcomes.
• Assist with discharge planning and denial management as needed.
• May require weekends, holidays, evenings, nights, and on-call with possible shift flexibility.
• Demonstrates Service Excellence at all times.
