Responsibilities

• Maintains competency in InterQual criteria.
• Collaborates with social work to address social issues affecting discharge planning.
• Documents and provides discharge planning services to optimize posthospital needs.
• Collaborates with the interdisciplinary team to transition the patient to the next level of care.
• Uses evidence-based practices to evaluate care and resource use.
• Develops relationships with community organizations for continued care.
• Coordinates with post-discharge providers to ensure safe discharge planning.
• Facilitates care conferences with patient/family and healthcare team.
