Qualifications

• Bachelor’s Degree in Nursing with additional training/certification in case management. BSN preferred.
• RN Indiana license. Certification in an area of case management is preferred.
• Three to five years of clinical nursing experience with leadership/management abilities. Critical Care experience is highly desirable.
• Demonstrates critical thinking, clear communication (verbal/written/listening).
• Competency in computer applications. May require the ability to speak Spanish.

Responsibilities

• Knows and adheres to laws and regulations pertaining to patient health, safety and medical information.
• Provides quality and continuity of care through collaboration with other departments for patients on assigned caseload.
• Provides leadership and effective communication to ensure outcomes for every patient are met; negotiates among team members and physicians.
• Conducts comprehensive team meetings regarding patient needs that affect cost, quality, and LOS; serves as central point of communication.
• Demonstrates customer service orientation in daily activities, communication, and interactions.
• Manages use of human, financial, and material resources; assesses plan of care and recommends changes to progress.
